Patients are asked to complete a registration form giving as much detail as possible. You will be asked for some proof of identity. From time to time the practice may apply to ‘close’ the patient list for new registrations. This is to ensure that we are able to maintain a high standard of care for our existing patients. This does not apply to new baby registrations and immediate family members. Please check at reception or contact the surgery for further information.
