If you live within our practice area you are more than welcome to register with us. The new patient registration involves completing a registration form (GMS1 form) and a health questionnaire. You can collect both forms to complete from the reception desk and our reception team will be more than happy to assist.
If you are unsure of whether you can register with the practice due to the area you live in, please contact the practice on 0121 553 4000 at less busier times from 11.30am onwards.
If you have a printer at home you can print off the forms and bring them into the surgery.
If you don’t have a printer, you can use our Online Registration Form
You will need to fill in boths forms so please make sure to download and print both.
Please note, that if you are out of our area, a member of our registration team will contact you to advise what to do next.
The new patient registration will involve a health check with the healthcare team this to ensure that required tests are up to date and that we have an accurate note of all repeat medications you are taking.
