If you live within our practice area you are more than welcome to register with us. The new patient registration involves completing a registration form (GSM1 form) and a health questionnaire. You can collect both forms to complete from the reception desk and our reception team will be more than happy to assist.
If you are unsure of whether you can register with the practice due to the area you live in, please contact the practice on 0121 553 4000 at less busier times from 11:30am onwards.
Please note, that if you are out of our area, a member of our registration team will contact you to advise what to do next.
The new patient registration will involve a health check with the healthcare team this to ensure that required tests are up to date and that we have an accurate note of all repeat medications you are taking.
New patient registration applies to everyone. The practice does not discriminate on the grounds of race, gender, soical class, age, religion, sexual orientation or apperance, disability or medical condition.
Changes to personal contact details
Please keep the practice up to date with any changes to your contact details, so we can keep you records up to date. This includes title,name, address and contact number. You can do this by completing a slip at reception which will then be changed by a member of our admin team.
